*I hereby authorize the veterinarian to examine, prescribe for, and/or treat my pet(s) and I agree to pay for all services authorized and provided. I understand that full payment for treatment is due at the time of service or when my pet is released. I assume full responsibility for all charges incurred for the care of all my pets on file with Garrett Veterinary Services. I also understand that a deposit mat be required for surgical treatment or hospitalization.
